An occasional TPK is fine. If you TPK or near-TPK them all the time, something is wrong. Worst case scenario, everyone dies and rolls new characters. This doesn't mean the end of the campaign. If the party does TPK, my advice is to toss the ball back to them. If they want a new party, let them decide how they will pick up the trail. Retcon things if you have to. When I’ve TPKed in the past, as a player at least, I tend to prefer scrapping the campaign altogether. Sometimes the bad guys win, and that can be a satisfactory conclusion.
